Pharmacokinetics Zirgan Gel:
In the ophthalmic use of the drug 5 times a day for 11 – 15 days in the treatment of superficial herpetic keratitis, the levels of ganciclovir in plasma are very low: an average of 0.013 ug / ml (0 = 0.037).
Ganciclovir penetrates quickly into the cornea and anterior eyes in a few hours of drug concentration in the anterior chamber of moisture exceeds the mean effective dose (ED50).
Pharmacodynamics Zirgan Gel:
Zirgan Gel – 9 – [(1,3-dihydroxy-2-propoxy) methyl] guanine, or DHPG is a nucleoside that inhibits the replication of human viruses herpes (herpes simplex of the first and second type, cytomegalovirus) and adenovirus serotypes following: 1, 2, 4, 6, 8, 10, 19, 22, 28.
In infected cells is converted to ganciclovir Zirgan triphosphate – the active form of the active ingredient.
Phosphorylation occurs predominantly in infected cells, the concentration of ganciclovir triphosphate in uninfected cells is 10 times smaller.
Antiviral activity of ganciclovir triphosphate to inhibit viral DNA synthesis by two mechanisms: competitive inhibition of viral DNA polymerase and direct inclusion into the viral DNA with blocking its elongation.
Indications for use Zirgan Gel:
Acute superficial keratitis caused by herpes simplex virus.
Dosage and administration Zirgan Gel:
Instilled into the lower conjunctival sac of the affected eye 1 drop 5 times a day to a full re-epithelialization of the cornea, then 1 drop 3 times daily for 7 days.
Duration of treatment should not exceed 21 days.
Side effects Zirgan Gel:
Common (> 1 / 10):
- A transient burning sensation or tingling in the eye.
Less common (> 1 / 100, <1 / 10):
- Superficial punctate keratitis.
ContraindicationsZirgan Gel:
Hypersensitivity to ganciclovir, acyclovir, or any component of the drug.

This medicine is not intended for the treatment of cytomegalovirus infection of the retina.
Efficacy against keratoconjunctivitis caused by other types of viruses has not been established.
No special studies in immunocompromised patients was conducted.
Because of the detected genotoxicity, men who use eye gel Zirgan Gel are advised to use local measures of contraception (condoms) during treatment and within three months after its completion. For women of childbearing age must use contraception.
Benzalkonium chloride may cause eye irritation and discoloration of soft contact lenses. Should not be allowed contact with the drug soft contact lenses. Remove contact lenses before using the product, and re-install them no sooner than 15 minutes after instillation.
Use in Pediatrics
The drug is not recommended for use before the age of 12 years.
Pregnancy and lactation
There is enough experience on the application of eye gel Zirgan Gel to assess its safety during pregnancy or breast-feeding. In this regard, it is not recommended to use the drug during pregnancy and lactation, except for the absence of alternative methods of treatment.
Features impact on the ability of driving and potentially dangerous machinery
